Team Member Application

I, the applicant, understand and agree that:

1.  Under company procedure, a routine inquiry may be made to provide information on my work record.

2. Employers for whom I have worked in the past may be requested to furnish Nu Wave Fish & Chicken's information concerning my past employment or activities,

3. Any false statement, omission or misrepresentation made in connection with my past record or any other part of my application for employment with Nu Wave Fish & Chicken's will be justification, should I be employed, for my dismissal regardless of when such fact may be discovered.

4. This position may require me to work various shifts, weekends and holidays and may require me to work a temporary assignment at another restaurant location or be transferred.

I agree that any missing or incorrect information will disqualify me as an applicant.
